Q: Is there a physical clinic I can go to?

A: Not at this time. ThriveWell is dedicated to removing barriers to care by offering safe, convenient medical and mental health services through in-home visits and telehealth—meeting patients where they are in today’s fast-paced world. 

 

We bring care to you through secure telehealth or in-home visits across Naples, Immokalee, and Ave Maria.

Q: Do I have to have a subscription plan to use your services?

A: No, a subscription is not required.
We offer flexible options to meet your needs—you can choose a one-time visit or enroll in a subscription plan for ongoing care at a lower monthly rate. It’s all about what works best for you and your family.

Q: What types of services does ThriveWell Medical Center offer?

A: ThriveWell provides in-home and telehealth primary care, pediatric wellness, mental health medication management, vaccinations, chronic condition management, school and sports physicals, and more.

Q: Who provides care at ThriveWell Medical Center?

A: All care is provided by board-certified nurse practitioners licensed in Florida, with experience in both primary and psychiatric care (and more).

Q: How do I book an appointment?

A: You can book online through our patient portal or call 239-920-3588. Same-day appointments may be available.

 

When you access either link (scheduling, patient portal), as a first time patient, you will need to fill out the registration form.

Q: What mental health conditions do you treat?

A: We currently offer psychiatric medication management for primary care such as anxiety and depression.

 

By the end of the year we plan to have comprehensive care for all mental health conditions—available via in-home or telehealth.

Q: Do you offer services for children?

A: Yes! We provide pediatric care, including wellness visits, sick visits, physicals, vaccinations, and mental health support.

Q: What is included in the subscription plans?

A: Depending on the plan, subscriptions include up to 4 telehealth visits or 2 in-home visits per month, with discounted rates and family payment caps.

Q: Can I cancel my subscription?

A: Yes. Just provide written notice 60 days in advance by emailing admin@thrivewellmc.com.
