This site is intended for individuals aged 18 or older. By using our site, you confirm that you are legally able to enter into a binding agreement. Parents or legal guardians may use the site on behalf of minors in their care.

2. Medical Services Disclaimer

ThriveWell Medical Center provides medical and psychiatric care through in-home visits and telehealth. Use of this website does not establish a provider-patient relationship. All care is provided only after a formal intake and consent process has been completed.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

3. Appointments and Subscriptions

Appointment scheduling is available through our secure patient portal. Patients may choose one-time services or enroll in a monthly subscription plan.

• Subscription plans auto-renew unless canceled with 60 days' written notice.

• Visit limits apply per plan. Unused visits do not roll over.

• ThriveWell reserves the right to change pricing or subscription offerings at any time with appropriate notice.

4. Payment and Insurance

We accept various payment methods including credit/debit cards and HSA/FSA cards. Patients are responsible for all fees at the time of service unless other arrangements have been made.

We are currently undergoing insurance credentialing. Insurance coverage will apply only after formal approval with each insurer.

5. Intellectual Property

All content on this website, including logos, branding, graphics, and written materials, is the property of ThriveWell Medical Center or its licensors. You may not reproduce, distribute, or use any content without prior written permission.

6. Privacy Policy

Our collection and use of your personal information are governed by our Privacy Policy, which is incorporated into these Terms by reference.
